    Forms:Application form in triplicate (one saved by the Registration & Receiving Station, one for the donor)

              one for the executor (families, relatives, friends, or work unit, neighborhood cadres )

3. Memorial Certificate：

    Certification : the Registration Station should send each registrant a memorial certificate, issued by Beijing Red Cross,when returning the application form

4. Receiving Procedures：

    Inform: After the decease of the donor ,executor shall obtain a death certificate in the hospital,and inform the Receiving Station in Peking Union Medical College(PUMC).

    Receive: After consultation, the body is received by automobiles sent by the Receive Station.

    Registration：1) Donation ID    2) Registration number     3) Copy of the death certificate

                  4) Date of death    5) Age           6) Gender

                  7) Date of donation    8) Name and contact information of the executor

                  9) A photo of the donor(1 inch) and a copy of the donor's ID card

    Issue：    1) Receipt Certificate

               2) Souvenirs

               3) Roadmap to the forest of steles
